*Documents to be attached to your application:

1. Self-Improvement Loan Application Form.
2. Direct Deposit Form.

Submit all of the above (no faxes) to your local
SISIP FS / Base Financial Counsellor or CFPAF loan Administrator.

*Additional financial information may be required at the discretion of your financial counsellor, or CFPAF loan administrator.

Loan funds will be deposited into the member's bank account directly from CFPAF .

1. Select the hand tool in the Adobe environment.

2. Position the pointer inside the form field, and click. The I-beam pointer allows you to type text. The arrow pointer allows you to select a radio button.

3. After entering text or selecting a radio button, do one of the following:

• Press Tab to accept the form field change and go to the next form field.

• Press Shift+Tab to accept the form field change and go to the previous form field.

• Click inside the next form field to enter or edit text.

4. To print the completed form, select legal size paper (8.5" x 14"), click the print button from either the Adobe environment or your browser window before leaving the application form.
